What Documents Will I Need to Submit my Child’s Application?

When submitting your application for a new Canadian Passport, you will need to include the following with your application:

  • Proof of the child’s Canadian Citizenship
  • A long form birth certificate from the Canadian province or territory where the child was born or a citizenship certificate
  • Proof of parentage or of legal guardianship
  • All documents related to the custody, decision-making responsibilities, access, parenting time, or mobility of the child, including any divorce judgment or court order
