Passport Forms and Requirements

You can apply for a new adult passport if ANY of the following conditions apply to you:

  • You do not have a valid Canadian passport.
  • Your passport’s issue date is over 15 years old.
  • Your current passport was issued when you were under 16 years old.
  • You need to update personal information on your current passport.
  • Your current passport is damaged and you do not have the issue date, expiration date and passport number.
  • Your current passport is lost or stolen and you do not have the issue date, expiration date and passport number.
  • Your current passport was valid for less than five years.

You can apply for a new adult passport if ANY of the following conditions apply to you:

  • Your current passport was issued within the past 15 years.
  • Your existing passport is valid for either five or ten years.
  • You were 16 years old or older when your current passport was issued.
  • You have the same name, date of birth and place of birth as you want on your renewed passport.
  • If your passport has been lost, stolen or damaged, you’ll need to provide the date it was issued, the expiration date and the passport number in order to renew.
  • When submitting your application to replace your lost, stolen, or damaged Canadian Passport, you will need to include the PPTC 203 form (Declaration concerning a lost, stolen, inaccessible, damaged, or found Canadian travel document) alongside your application.
  • It can be printed on your most recent passport or have the observation sticker.

Applying for a child passport involves different requirements compared to an adult passport. 

  • A legal parent or guardian must act as the applicant on behalf of the child.
  • The application requires a guarantor to confirm the identity of the child and the applicant.
  • There is no renewal process for child passports. A completed child application must be submitted each time.

Replacing a lost, stolen, or damaged passport in Canada involved specific steps.
Before you apply, you’ll need to determine whether you qualify for a passport renewal or if you'll need to apply for a new passport.

You can apply for a passport renewal if your passport is an adult passport, was issued within the last 15 years, and you HAVE the following information:

  • Date that the passport was issued
  • Expiration date for your passport
  • Passport number

To apply you will need to submit all supporting documents required for a passport renewal by the Government of Canada.Child passports are not eligible for renewal.

The PPTC 203 - Declaration Concerning a Lost, Stolen, Inaccessible, Damaged, or Found Canadian Travel Document is required when reporting a lost, stolen, inaccessible, or damaged Canadian passport. This declaration must be submitted alongside your passport application for a new passport or a passport renewal, depending on eligibility requirements. The form must be completed online. Fillable/savable PDF forms have been tested for compatibility with Adobe Acrobat and are not supported.

  • Length of the face needs to be between 31 mm (1 1/4 inches) and 36 mm (1 7/16 inches) from chin to natural top of head.

  • Photo may be in color or black and white.
  • Provide on the back of one of the photos:
    Photographer must clearly write or stamp the date the photo was taken and the name and complete address of the photo studio
  • For New Passports and Child Passports: the Guarantor must clearly write “I certify this to be a true likeness of (applicant's name)” for an application and sign the back of the photo
  • When applying for a new adult passport, you must provide original proof of Canadian citizenship. Photocopies, even if signed, are not accepted by the Passport Program.
  • The original proof of citizenship document will be returned to you after verification. If you apply in person, the document may be verified and returned immediately.
  • A valid copy of ID that has been issued GREATER than 6 months ago must be included (Front & Back Copy). If you apply in person, the document may be verified and returned immediately.
  • ID can be a Drivers License, State ID, Military ID or any Gov't ID. Paper, Learner's Permits and temporary licenses are not acceptable.
  • The amount of the fee will depend on the specific validity of the passport. Make the cashiers cheque or money order payable to “Receiver General for Canada”. The fee may also be paid by credit card.
  • 5-year adult passport (age 16 or over): $120
  • 10-year adult passport (age 16 or over): $160
  • Personal cheques are not accepted.
  • The most commonly used itineraries are airline, cruise and hotel reservations.
  • A business letter on original company letterhead can be used in lieu of a travel itinerary.
  • The travel itinerary and application dates must match and include the applicant’s name.

  • It can not be renewed if it is Damaged or Mutilated, unless the issue and expiration date is clearly legible and the passport is returned.
  • *It cannot be renewed if it has been lost or stolen, unless the issue, expiration date and passport number are all known and can be added to the renewal application.

Child Passport

  • If your child has yet to receive a Social Security #, you must show documented paperwork that one has been requested.
  • Complete and print 1 copy (Single-Sided Only) of the Online Government Passport Application Form - PPTC 155.

  • Ensure the bar code is located at the top right-hand corner of page 1.

  • Proof of parentage; Must be an ORIGINAL or CERTIFIED copy with a raised or embossed seal of an Canadian Birth Certificate (with parent[s] names on it). If a parent’s name has changed since birth, a Marriage Certificate or other document showing the legal name change must be submitted.
  • The amount of the fee will depend on the specific validity of the passport. Make the cashiers cheque or money order payable to “Receiver General for Canada”. The fee may also be paid by credit card.
  • 5-year adult passport (age 16 or over): $120

Proof of Identity

If you’re applying for a new passport rather than renewing one, you must submit at least 1 valid document to support your identity:

  • You can submit the original or a photocopy of both sides.
  • Remember that each photocopy must be signed and dated by your
  • Guarantor or
  • Signing official, if you used a Statutory Declaration in lieu of a Guarantor form
  • Additional documents if you’re changing your name, if applicable and passport number.

If your passport is still valid, you must include it with your application. In some cases, you can use your Canadian passport as your identification document (ID).
If your passport has a valid visa in it:

  • Tell us on your form if you need the visa and want your original passport back. If you don’t, the visa could be damaged when the previous passport is canceled.

Requirements for your ID

To support your identity, your ID must

  • Be valid
  •  Include your:
  • Name
  • Date of birth
  • Photo
  • Signature
  • Be issued by a federal, provincial, territorial or state government authority, or the equivalent abroad

Types of ID you can use:

You can use any of the following types of ID alone or in combination to meet the above requirements:

  • Canadian passport
  • Driver’s license issued by a province or territory, or the equivalent abroad
  • Health card issued by a province or territory, or the equivalent abroad
  • Certificate of Indian Status
  • Military ID
  • Foreign passport
  • Non-driver’s license issued by a province or territory, or the equivalent abroad
  • ID card issued by the federal government, or a provincial or territorial government, or the equivalent abroad
  • Employee identification issued by the federal government, or a provincial or territorial government, or the equivalent abroad.

You cannot use your social insurance number (SIN) card as ID.
If your ID isn’t in English or French, you must provide an official translation of it.

Proof of Citizenship

Proof of Canadian Citizenship for New Adult Passport Applications

When applying for a new adult passport, you must provide original proof of Canadian citizenship. Photocopies, even if signed, are not accepted by the Passport Program.

The original proof of citizenship document will be returned to you after verification. If you apply in person, the document may be verified and returned immediately. Proof of citizenship is not required for passport renewals.

If You Were Born in Canada
Submit one of the following:

  • A birth certificate (long form or short form) from the province or territory of your birth
  • A Canadian citizenship certificate

Important Notes on Laminated Documents
Laminated proof of citizenship documents may not be accepted unless laminated by the issuing provincial, territorial, or federal government agency. Documents stating “void if laminated” or those not originally laminated are unacceptable because lamination can obscure the document’s original security features necessary for validation.

Obtaining New Proof of Canadian Citizenship

  • If born in Canada, apply for a birth certificate.
  • If born outside Canada, apply for a citizenship certificate.

Proof of Citizenship

A citizenship certificate is an official document that verifies your Canadian citizenship. It is not a travel document. A Canadian passport is the only reliable and universally accepted travel and identification document available to Canadians for the purpose of international travel.

Electronic Certificates (e-certificates)

The e-certificate is the digital format of the citizenship certificate and serves as valid proof of Canadian citizenship. It includes a certificate number that starts with an “X,” which is the only difference from a paper certificate where the number starts with a “K.” The e-certificate also contains your unique client identifier (UCI), your family name and given name(s), your date of birth, your gender (male, female, or another gender), and the effective date of citizenship.

Paper Certificates

A paper certificate is an 8½ x 11-inch document that serves as valid proof of Canadian citizenship. It includes the same information as an e-certificate: the certificate number that starts with a “K,” which is the only difference from an electronic certificate where the number starts with an “X.” The paper certificate also contains your unique client identifier (UCI), your family name and given name(s), your date of birth, your gender (male, female, or another gender), and the effective date of citizenship.

Citizenship Cards

Issued citizenship cards and the commemorative certificates that accompanied them ceased in February 2012. However, if you have a citizenship card, it is still valid proof of Canadian citizenship. 

The date on your citizenship card is not the date you became a Canadian citizen; it is the date the card was produced. To update or replace your citizenship card, you will need to apply for a citizenship certificate.

Other Acceptable Proof of Citizenship

Other acceptable proofs of Canadian citizenship include:

  • Birth certificates from a Canadian province or territory (with some exceptions)
  • Naturalization certificates issued before January 1, 1947
  • Registration of birth abroad certificates issued between January 1, 1947 and February 14, 1977
  • Certificates of retention issued between January 1, 1947 and February 14, 1977.

Proof of Travel

If you need your passport within 1-9 business days, your travel should be planned no sooner than 25 days from the date of submission. You must provide proof of the necessity for the passport. Acceptable forms of proof include:

  • An airline, bus, or train ticket
  • A written statement explaining why you need to travel if traveling by car
  • A travel itinerary showing proof of payment
  • Proof of illness or death in the family requiring travel
  • A written statement from you or a third party explaining the need for the passport (e.g., for legal, banking, or residency requirements)

Please note, if we are unable to deliver the passport to you or your child in time, we are not liable for any losses incurred. We recommend not finalizing any travel plans until you have received your passport.

When Should I Plan Travel?

Canadian citizens are suggested to plan travel no fewer than 25 business days after submitting documents for processing.

Company Letter

For travelers who cannot obtain a travel itinerary, the Canada Passport Agency will accept a company letter to be used in the place of airline tickets or a travel itinerary.

Please type this information onto your company letterhead and make sure your departure date in the letter is within 14 days.

This letter must be signed by a company representative. The signature must be original - no copies, scans or faxes can be used. You cannot sign your own letter.

Government Passport Fees and Payment

The Canadian government accepts specific payment methods for passport applications. Please ensure that your payment follows these guidelines to avoid delays or suspensions of your application:

General

A passport is an officially recognized government document that certifies one's identity and citizenship, permitting a citizen to travel abroad and to re-enter Canada.

Regular processing through the Passport Program takes an average of 10 to 20 business days, plus mail time. You can apply in person at a passport office or Service Canada Centre by appointment or walk-in, or by mail. Express service through the passport office takes 2–9 business days, plus extra fees. You can apply in person at a passport office with express pick-up service by appointment or walk-in. Urgent service is available by the end of the next business day, plus extra fees. You can apply in person at a passport office with urgent pick-up service.

Most major cruise lines strongly recommend that all guests travel with a passport. Passports make it possible for you to fly back to Canada from a foreign port should there be an emergency. A passport also allows you to fly from Canada to and between foreign ports, should you miss your scheduled port of embarkation.

No. If you want to change your name, date of birth, place of birth or gender identifier on the new passport, you must apply for a new adult passport.

Your no-fee military (brown) passport can only be used when traveling overseas in discharge of your official duties. For personal travel, you must use a regular-fee tourist (blue) passport.

We guarantee that we will submit your application for processing. However, there are circumstances beyond our control that can delay or suspend your application. In addition to weather, examples include owing child support or back taxes, travel restrictions due to a felony, or lacking the necessary documentation.

There are a couple of ways you can get a certified copy of your birth certificate in Canada:
You can order your birth certificate online through your provincial or territorial Vital Statistics office.

Call or visit the Vital Statistics office in the province or territory where you were born. You can usually present an ID to get a certified copy or they can mail a copy to you

Passport applications may be denied or rejected for several reasons. The most common reason is that the application is missing the necessary documents, such as an original birth certificate, proper photographs, or the correct fee. 

Additionally, passports may be denied for the following reasons: outstanding child support, unpaid federal taxes, an outstanding felony/criminal warrant, or a prior felony conviction that prohibits international travel. Please be advised that the Canadian government has the right to request additional documents at its discretion.

If you claim Canadian citizenship through the naturalization of your parent(s), submit the Certificate(s) of Naturalization of your parent(s), your foreign birth certificate, and proof of your admission to Canada for permanent residence.

Yes, you can! You must submit your original naturalization certificate with your application. Your documents will be returned by the agency with your new passport. Supporting documents may be mailed separately from your passport.

Passport Renewal

Yes, you have five years from the expiration date to renew your passport, as long as it was a five or 10-year passport. (This does not include temporary, military or child passports).

The expiration date is located on the data page where your full name and photo are also displayed. Look for the second to last piece of information next to your picture. The title should be “Date of expiration/Date d’expiration/Fecha de caducidad.” Under that phrase, you will see a date in the format of day/month/year. This is the expiration date of your passport.

Child passports (issued when the child was under 16) are only valid for 5 years. They cannot be renewed in the same way as an adult passport. You must apply for a new child passport until the age of 16.
If the minor is over the age of 16, you must apply for an adult passport. Ensure you provide all necessary documents, including proof of citizenship, proof of identity and the required fees.

It is recommended that Canadian citizens renew their passports approximately nine months before the expiration date, as many countries require at least six months of validity remaining on your passport.

No, new passport photos must be taken each time a passport is issued. Photos must be no older than six months, and the Canadian government is very strict on this policy. Photos must reflect your current physical appearance and as this may change over time, passport photos must be updated each time you apply.

Lost/ Stolen Passport

No, you must have your valid passport in your possession. 

No, you cannot use a photocopy of your passport for international travel. You must have a valid passport book in your possession when you travel.

Lost and stolen passports are able to be renewed if Canadian citizens have the issue, expiration date and passport number. You will need to report it lost and include a declaration concerning a lost, stolen, inaccessible, damaged or found Canadian travel document.

If the issue, expiration date and passport number is not available, a new passport must be applied for.

Passports reported lost or stolen are immediately invalidated and can no longer be used for travel.

If your passport or travel document is lost or stolen, Canadian government offices abroad can issue you an emergency travel document or a temporary passport if you are in urgent, proven need and are stranded in a foreign country.

You will follow the instructions for a new passport and will also be required to submit an original or certified copy of your marriage certificate or divorce decree which proves your legal name change. Child Passport

Any Canadian citizen, no matter what age, traveling outside Canada by air requires a passport. This includes infants and newborns. Rushmypassport.ca can assist if you need to get a new child passport or if you need to update a child’s passport.

Yes, if your child is too young to sign the passport application, all parents or legal guardians must sign the application on their behalf. When submitting the application by mail or visiting a Service Canada Centre, the parent(s) will need to sign the document along with a guarantor.

Acceptable passport photos for babies must be done in the size of 50 mm wide by 70 mm high (2 inches wide by 2 3⁄4 inches high) and taken within the last 6 months. Your baby must be facing forward, looking at the camera and set against a white or off-white background. If your child is too young to sit up, the photo can be taken with your baby lying on a white blanket. Please try to get a photo with your baby's eyes open, and ensure your baby is not screaming or crying. hats and head coverings worn daily for religious beliefs or medical reasons are allowed just make sure your child’s full face is clearly visible. No hats, headgear, baby bonnets or pacifiers are allowed. The baby must be the only thing in the picture. Nothing used to support the baby can appear in the picture. That includes you, your hands or arms, or your baby’s car seat.

To apply for a child passport, you must be one of the following: one of the child’s parents (if you’re not separated or divorced), the parent who has custody or decision-making responsibilities for the child (if you are separated or divorced) or the child’s legal guardian. You must include all legal documents proving your legal guardianship of the child. All parents or legal guardians should sign the application form.

When applying for a Canadian passport, if parents have joint custody or joint decision-making responsibilities, either parent can apply for the passport, however all parents or legal guardians must sign the application on their behalf. If you don’t know where the other parents or legal guardians are, you may contact the Government of Canada for assistance.
