How to Replace a Lost, Stolen, or Damaged Canadian Passport

Replacing a lost, stolen, or damaged passport in Canada involves specific steps.
Before you apply, you’ll need to determine whether you qualify for a passport renewal or if you’ll need to apply for a new passport.


You can apply for a passport renewal if your passport is an adult passport, was issued within the last 15 years, and you HAVE the following information:

  • Date that the passport was issued
  • Expiration date for your passport
  • Passport number

To apply you will need to submit all supporting documents required for a passport renewal by the government of Canada. Child passports are not eligible for renewal.

If your passport is lost, stolen, or damaged and you do not meet the requirements for a renewal, or DO NOT HAVE the following information from your current passport:

  • Date that the passport was issued
  • Expiration date for your passport
  • Passport number

If you are replacing a lost, stolen, or damaged passport for a minor, you will need to apply for a child passport. Child passports are not eligible for passport renewals.

How do I report that my passport has been lost, stolen, or damaged?

When submitting your application to replace your lost, stolen, or damaged Canadian Passport, you will need to include the PPTC 203 form (Declaration concerning a lost, stolen, inaccessible, damaged, or found Canadian travel document) alongside your application.

This form serves to officially report the status of your previous passport. It is used to declare that your passport has been lost, stolen, inaccessible, damaged, or found. The declaration helps prevent misuse of your passport and facilitates the issuance of a new document.
